During the 2019 Plan Year, look forward to the ‘modeled’ Ascend to Wholeness adapted into our HMO Plans, as we will offer two Plans: one with the continued wellness program and one without. To continue your eligibility for the Traditional Plan, please actively participate in the Kaiser Wellness Program during the 2019 Plan Year.
The TRADITIONAL PLAN offers a one-stop shop Health Maintenance Organization (HMO) for medical and prescription needs. Members can log on to kp.org to see their medical record, make appointments, find a provider, email their provider and more. A new feature to the plan is the Kaiser Wellness program, which members will be encouraged to participate in during the 2019 plan year.

PRINCIPAL Life offers three levels of benefits within its plan design for dental coverage - EPO, PPO and Non-network. Members can log onto www.principal.com to obtain their benefit summary, claim activity and other member services.
Principal Dental Coverage - Point of Service Summary
VSP CHOICE provides the highest level of eye care, including WellVision Exam, choice in eyewear, plus extra savings.
As a member, log onto vsp.com to review your benefit information and find an eye care provider that is right for you.
VSP Vision Care - Benefits Summary
Kaiser Permanente is an HMO option. Kaiser facilities are located throughout the North/East Bay regions and the Sacramento/Stockton region. Vision care is provided through VSP. Dental is provided through Principal Dental.
